The Mount Pleasant hospital was overcrowded within years of opening, and in 1868 the legislature appropriated funds for a new institution west of Independence. Construction took a decade and cost $845,000; the first patients were admitted on May 1, 1873. The main building followed the era's approved corridor plan associated with the Kirkbride model — designed by Stephen Vaughn Shipman, who drew asylums for Illinois, Wisconsin, and Minnesota — with walls of stone quarried around Farley and Stone City on a foundation of prairie granite.

The campus grew with mansard roofs, domes, and a tunnel network linking its buildings, and its name tracked the century's changing vocabulary: Hospital for the Insane, State Hospital, then Mental Health Institute.
