The record — long form

Pilgrim Psychiatric Center was the largest hospital in the world — a city built for the sick on a thousand acres of Long Island farmland. It opened in October 1931 with 100 patients transferred from Central Islip; nine months later it held 2,018. By 1954 it held 13,875, with more than 4,000 employees — a census no hospital anywhere has ever exceeded. It had to be a city: its own power plant, waterworks, fire and police departments, courts, post office, church, farm, swinery, and its own cemetery for the patients nobody claimed.

Inside that city was Building 23 — the lobotomy building. From the 1940s into the 1970s, thousands of patients went into its operating rooms for the 45-minute procedure that drove an instrument through the skull and severed the prefrontal cortex. Life magazine had called Pilgrim 'A City of the Insane' in 1938; by mid-century it was also a city of the operated-on, where rest and farming gave way to electroshock, insulin comas, and the ice pick's quieter cousins.

The city shrank as fast as it grew. Pharmaceuticals emptied the wards; Kings Park and Central Islip's survivors were folded into Pilgrim in 1996, and today the campus runs at a few hundred beds amid derelict buildings and its own potter's field.
